Enrollment Process
We work on a first-come, first-served basis.
Priority Enrollment
PRIORITIZED PATIENTS
Those prioritized in the enrollment process include:
✓ Pregnant Women
✓ Patients Who are Hospital Transfers
✓ Patients Who Transfer from Another OTP
✓ Patients Who are Coming from Correctional Facilities
STEP BY STEP
Steps of the enrollment process
Patients will not be denied services based on finances.
1 The patient must provide a copy of their identification and proof of payment.
2 The patient must provide a urine sample. It must be positive (+) for opioids and negative (−) for benzodiazepines.
3 The patient must complete a financial contract.
4 The patient must complete medical documents prior to meeting with the provider.
5 The patient must complete the intake, Alaska Screening Tool, and Client Status Review forms.
6 The patient must complete a medical assessment.
7 The patient must complete an enrollment assessment and treatment plan.
